To ensure that you are ready to treat your clients at Emporium Treatment Clinic the registration must be completed and the following items must be sent to the clinic:

• Signed relevant licence (set day/s or ad hoc)
• Copies of your qualifications
• Copies of your professional body membership
• Copy of your insurance
• Proof of ID
• Proof of address
• Annual Registration Fee of £100
